  1. All Freedom and Clear Choices locations can accept any insurance if the patient also has Medicaid.
  2. Medicaid covers for Freedom based upon the county where the patient is registered. This not necessarily the county where the patient resides.
  3. If you have a patient registered in a non-listed county, contact Melissa. She will obtain a single case agreement for the patient.
  4. If the patient has Medicaid, then requirement for licensed therapist is not longer required.
  5. If you receive a call from a customer with an insurance that is not listed, but the card indicates a PPO, please contact Melissa. In most cases we can accept the patient but we want to verify their out of pocket expense before they begin care.
  6. If the patient has Medicaid, then requirement for licensed therapist is not longer required.
